How We Help

We focus on the whole person and their unique situation. Services may include:

  • Case plan support & reunification services

  • Referrals to mental health and substance use providers

  • Housing navigation and relocation assistance

  • Medicaid enrollment support

  • School enrollment and IEP/504 advocacy

  • Coordination with DCF, legal teams, and group homes

  • Support for caregivers, foster youth, and system-involved families

  • Life skills coaching (budgeting, daily routines, parenting strategies)

Every client receives a personalized Strengths & Needs Assessment, a Wraparound Service Plan, and a dedicated case manager to walk alongside them—not in front of them, not behind them, but beside them.

At Rooted Futures Network, we provide Targeted Case Management (TCM) and wraparound support services to youth, adults, and families navigating systems that often feel overwhelming, disconnected, or unfair.

We specialize in helping people connect the dots—between services, supports, and systems—so they can move forward with structure, advocacy, and a real plan.

  • Youth in or aging out of foster care

  • Kinship caregivers and legal guardians

  • Adults at risk of losing housing or stability

  • Individuals with Medicaid who are navigating behavioral health or support systems

  • Families impacted by court involvement, school system barriers, or incarceration
